Privacy Policy

The short version

This policy covers censusrise.com and the community-branded care assessment and tour booking pages we operate at ourcareguide.com. If you followed a privacy link from a care assessment, this is the policy that governs it.

We collect what we need to book tours and help families find the right community — nothing more. We do not sell your personal information, and we do not share it with third parties for their own marketing.

Mobile phone numbers and text-message consent are never shared with anyone for marketing purposes, and are never sold. If we text you, you can stop it at any time by replying STOP. The details are in Section 5.

01Who we are

CensusRise is a service of For Impact Co, LLC (“CensusRise,” “we,” “us,” or “our”). We build and operate care-assessment and tour-booking systems for senior living communities.

This policy explains what personal information we collect, why we collect it, who we share it with, and the choices you have. It applies to censusrise.com, to ourcareguide.com and the community-branded care assessment and tour booking pages we operate there on behalf of our client communities, and to the email and text messages we send through those systems.

02Who this policy covers

Two different groups of people give us information, and they give us very different things:

  • Senior living communities and their staff — owners, executive directors and sales teams who contact us, book a call, or use the CensusRise dashboard as a customer.
  • Families researching care — people who complete a care assessment, book a tour, or exchange messages with us through a page we operate for a community.

Care assessment and tour booking pages are hosted at ourcareguide.com and are branded for the community you are researching. Even though those pages carry the community’s name and look, the underlying system is operated by CensusRise on that community’s behalf, and this policy — not a separate one — governs how we handle the information collected there. We share what you submit with the community you contacted — that is the entire point of the assessment — and that community then handles your information under its own privacy practices.

03What we collect

Information you give us directly

  • Contact details — name, email address, phone number, and the community or organization you are associated with.
  • Assessment answers — who you are finding care for, your general situation, the kind of day-to-day help that person needs, your preferred area, and your timeline.
  • Scheduling information — the tour or call you booked, and any notes you added.
  • Messages — the content of texts, emails, and chat messages you exchange with us or with a community through our system.
  • Customer account information — for community customers, billing contact details and the configuration you provide (pricing, care offerings, staff contacts). Card payments are handled by our payment processor; we do not store full card numbers.

Information about someone else

Many people complete an assessment on behalf of a parent, spouse, or other relative. If you give us information about another person, please only share what is necessary, and only if you are entitled to share it. We treat that information the same way we treat your own.

The assessment asks about general daily-living needs so a community can point you toward the right level of care. It is not a medical evaluation, we do not ask you for medical records or diagnoses, and the result is guidance about where families in similar situations usually start — not a clinical or care-level determination.

Information collected automatically

  • Device and usage data — IP address, browser type, pages viewed, referring page, and timestamps.
  • Cookies and similar technologies — see Section 8.
  • Message delivery data — whether an email or text was delivered, opened, or replied to, so we can tell whether the system is working.

04How we use information

  • To deliver your assessment result and connect you with the community you contacted.
  • To schedule, confirm, and remind you about tours and calls.
  • To answer your questions and follow up about your inquiry, by text or email, until you tell us to stop.
  • To alert community staff that a family is waiting to hear from them.
  • To operate, support, secure, and improve our services, including measuring whether tours actually get booked.
  • To provide our community customers with reporting about their own leads and results.
  • To send business communications to community customers and prospects about our services.
  • To meet legal, tax, and regulatory obligations, and to enforce our terms.

We do not use assessment answers to build advertising profiles, and we do not run advertising targeted at you based on the care needs you describe.

05Text messaging (SMS) and your consent

When you provide your mobile number and agree to be contacted — by checking the consent box on a care assessment or tour booking form at ourcareguide.com or on a page we operate for a community — you are opting in to receive text messages from us on behalf of the community you contacted. We keep a record of that consent, including when and on which form you gave it. We do not add anyone to text messaging by any other means, and we never obtain mobile numbers from purchased lists, data brokers, or referral services.

How oftenMessage frequency varies based on your inquiry and how you reply. You are never enrolled in a recurring broadcast list.

Consent to receive text messages is not a condition of any purchase, of touring a community, or of using our services. Opting out of texts does not opt you out of email; those are separate, and you can unsubscribe from email using the link in any email we send.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors who provide support services, such as customer service and message delivery, is permitted.

All of the sharing categories described in this policy exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

We do not sell mobile phone numbers or SMS consent data, and we never share them with data brokers, lead resellers, or referral services.

The community you contacted receives your inquiry so its staff can respond to you directly. That is the service you asked for, and it is not a sale or a marketing transfer of your opt-in data.

06The Care Guide and automated messages

Some of the messages you receive are sent by an automated digital assistant we call the Care Guide. It identifies itself as a digital assistant, and it is there so that questions asked at ten at night do not sit unanswered until Monday.

To generate replies, the content of your conversation is processed by an artificial-intelligence service provider acting on our instructions under contract. That provider is not permitted to use your information for its own purposes.

The Care Guide answers only from information the community has given us, such as pricing, care offerings, and availability. It does not provide medical, legal, or financial advice, and it does not make care-level determinations. You can ask for a human at any time and we will hand you to the community’s staff. No decision that produces a legal or similarly significant effect about you is made by automated means.

07How we share information

We do not sell your personal information. We share it only as follows:

  • With the community you contacted — your assessment answers, contact details, and booking so its staff can reach you. This is the purpose of the service.
  • With service providers who operate the system under contract: hosting and application infrastructure, email delivery, text-message delivery, calendar scheduling, payment processing, and the AI provider described in Section 6. They may use your information only to perform services for us.
  • For legal reasons — when required by law, subpoena, or valid legal process, or to protect the rights, safety, or property of any person.
  • In a business transfer — if CensusRise is involved in a merger, acquisition, financing, or sale of assets, information may transfer as part of that transaction, subject to this policy.
  • With your direction — anyone else you ask us to share with.

Every category above excludes text messaging originator opt-in data and consent. Mobile opt-in information is not shared with third parties or affiliates for marketing or promotional purposes under any circumstances.

08Cookies and tracking

We use cookies and similar technologies to keep the site working, remember your progress through an assessment, and understand which pages families actually use. Some are strictly necessary; others help us measure performance.

Our client communities run their own advertising, and a community may place its own measurement tags on pages we operate for it. We configure those to record that an assessment was started or a tour was booked. We do not pass the substance of your assessment answers — the care needs you described — to any advertising platform, and we do not build advertising audiences from them.

Most browsers let you refuse or delete cookies in their settings. Blocking strictly necessary cookies may stop parts of the site from working. We do not currently respond to browser Do Not Track signals, because no common standard for them exists.

09How long we keep information

We keep personal information for as long as needed for the purposes described here: to serve your inquiry, to provide reporting to the community, to meet legal and tax obligations, and to resolve disputes. When information is no longer needed, we delete it or de-identify it.

Records of an opt-out are the exception. We keep those for as long as necessary to make sure we do not message you again.

10How we protect information

We use encryption in transit, access controls, and reputable infrastructure providers, and we limit access to the people who need it. No method of transmission or storage is completely secure, so we cannot guarantee absolute security. Please do not send us medical records, financial account numbers, or government identification numbers — we do not need them and do not ask for them.

11Your choices and rights

  • Stop texts — reply STOP to any message.
  • Stop emails — use the unsubscribe link in any marketing email. We may still send transactional messages, such as a confirmation for a tour you booked.
  • Access, correct, or delete — ask us for a copy of the personal information we hold about you, ask us to fix it, or ask us to delete it.
  • Withdraw consent — where we rely on consent, you can withdraw it at any time.
  • No discrimination — we will not treat you differently for exercising any of these rights.

Depending on where you live, you may have additional rights under state privacy laws, including the right to know what we collect, the right to delete, the right to correct, and the right to opt out of sale or targeted advertising. We do not sell personal information and we do not use it for cross-context behavioral advertising. To exercise any right, email [email protected]. We will verify your request before acting on it, and we will respond within the time the applicable law requires.

If you submitted an assessment to a community, that community holds its own copy of your information. You may need to contact it directly as well; we will tell you who to contact if you ask.

12Children’s privacy

Our services are for adults. We do not knowingly collect personal information from anyone under 18. If you believe a child has given us information, contact us and we will delete it.

14Changes to this policy

We may update this policy from time to time. We will change the “Last updated” date at the top, and if the changes are significant we will provide a more prominent notice. Continuing to use our services after an update means you accept the revised policy.
